Located in Medicine Hat, the Geriatric Assessment Team provides supports to clients age 65 and older with health problems related to getting older. Clients under the age of 65 are assessed on an individual basis. The team provides assessment, counseling, treatment, and education to clients with complex health needs living in the community. When necessary, referrals may be made to other community agencies.
Referral must be made by Home Care or by a physician.
