Provides psychosocial service, support, and resource information for people in hospital, outpatient programs, and some community care and supported living facilities. Also provides support and resources to promote physical and mental well-being, while also addressing social needs. Helps clients who run into problems because of illness, disability, and/or hospitalization, which may interfere with their being able to get the most benefit from their medical treatment.
Clients are registered as inpatients in an AHS facility or program.
