About Us 

Primary Care Networks (PCNs) use a collaborative team approach to provide care for patients and to coordinate primary health care services provided by family physicians, Alberta Health Services and other health care professionals.
PCNs are a way to provide team support for family doctors by bringing teams of health professionals together – doctors, nurses, dietitians, social workers, mental health professionals, pharmacists, occupational and respiratory therapists and others.

What is a PCN?
A PCN is not necessarily a bricks-and-mortar building – it is a “network” of family physicians and health care providers working together to provide primary care to patients. A PCN may be composed of one clinic with many physicians and support staff, or several physicians in several clinics across a geographic region.

Each network is unique and has the flexibility to develop programs and provide services that meet the specific needs of its patients. This grassroots approach allows – and encourages – the PCN to focus on the needs of the local patient population. The Primary Care Initiative oversees and supports the development of PCNs.

Palliser PCN
The Palliser Primary Care Network provides services for patients in south-eastern Alberta. A group of more than 50 family physicians and other health care professionals work together to improve primary care for our patients.

Physicians participate in interdisciplinary family practice teams / programs which have other health care providers working out of physician clinic locations to collaborate with physicians in the delivery of services to chronic / complex care patients.

Chronic Disease Management
Based on your medical history your physician has determined that your care can be enhanced by the addition of another health care professional to your primary care team. Initial appointments with another health care professional may take as long as one hour.  Follow-up appointments may take as long as 30 minutes.

 

Who is on my Healthcare Team?
Physicians, Registered Nurses, Dieticians, Physiotherapists and Counsellors may be part of your primary care team. As a member of the PCN your health care provider(s) will remain abreast of best/promising practices and current guidelines in chronic disease management.

You can expect a thorough assessment of your condition, health counseling, education, and referrals as part of the care you receive from another health professional.

Your physician and other health care professional will be working closely together to improve your health.
